With value-based care (VBC) programs rolling full steam ahead, the need for acute and post-acute care (PAC) collaboration is more critical than ever. In order for nursing facilities to boost revenue and operate successfully, they must work in conjunction with other stakeholders in the patient journey to develop and implement innovative strategies that reduce avoidable hospital readmissions, better execute successful discharge strategies, improve quality of care, and drive better patient outcomes.
