Providers have struggled with medical record documentation for as long as there have been medical records. It is a common concern and one that is not limited to nursing documentation. It is also a concern that can result in state survey deficiencies, reduce reimbursement and legal liabilities. This session will review some documentation common concern areas in the medical record as well as provide some considerations for systems to address medical record documentation.
